The Department of Communities (we, our, us-Department of Communities ABN 41 889 424 592) is a Western Australian Government human services department that brings together vital services and functions that support individual, family and community wellbeing. The Department of Communities' partners across government and the community services sector to support diverse communities throughout Western Australia including to services to support persons wanting and needing homelessness services. Find My Way (the Portal) provides reliable, real-time homelessness support services data, is intuitive, easy to use, and is a secured platform. The comprehensive and searchable directory aims to improve visibility and access to accommodation and other services for people sleeping rough and experiencing homelessness.
Communities have engaged Infoxchange Australia (ACN: 098 019 544 / ABN: 74 457 506 140) (the 'Contractor') to undertake the design, development, support, and maintenance of the Find My Way web page portal ('this Portal').
Any personal and sensitive information you provide to us using this Portal will be used for the purpose of providing homelessness services to you. This information could include your name, age, telephone number, e-mail address, and postal address. We will use your personal and sensitive information to link you to suitable homelessness service providers. We will only use your personal and sensitive information for that purpose. If you do not provide us your personal or sensitive information, we may be limited in providing the help you may need.
Your personal and sensitive information that you share with the Portal will be collected and stored in a secure Portal platform. When your information is shared with homelessness services, this will be done using industry standard AES 256 encryption technology, both in transit and at rest. This will ensure that communication between Infoxchange, a business who will store your information, and services remain secured at all times. In addition, robust firewall technology protects Infoxchange's internal systems and users' information against intrusion via the Internet.
We will only provide your personal and sensitive information to relevant homelessness services that state their obligations in protecting confidential information. Through continuous monitoring and improving of our procurement processes we ensure that there is adequate coverage of information security clauses in supplier contracts and ensure alignment with the requirements of Communities and Infoxchange procurement standards.
Using this Portal is voluntary, and you may opt out of using this Portal at any time. You do not need to provide your personal or sensitive information to Infoxchange or us if you do not wish to do so. If you do not provide your personal or sensitive information to us or Infoxchange, Communities will still be able to provide the help you need to access homelessness services. All data collected using this Portal will be aggregated (including all personal and sensitive information) and de-identified prior to any reporting. What this means is that you will not be able to be personally identified in any reporting that Communities does on the Portal. Following the end and/or termination of the contract with Communities, the platform will be destroyed according to the vendor's data disposal authority.
Infoxchange has partnership agreements with partners who are required to sign a binding non-disclosure agreement that Portal data will only be shared for very specific purposes - with agencies and practitioners who provide services in health, welfare, and community support services and to allow networks of cooperative service delivery and development of products and services (“Other Service Providers”). Infoxchange may disclose your personal and sensitive information to Other Service Providers. All Other Service Providers agree to use all personal or sensitive information provided to them by Infoxchange from this Portal only for purpose of providing those products and services to Infoxchange and all Other Service Providers agree to keep all the personal and sensitive information from this Portal secure and confidential.
Infoxchange may also disclose information (including personal and sensitive information) when required to do so by law, for example, in response to a court order, a subpoena or to fulfil any other legal obligation, in response to a law enforcement agency's request, or in special cases when Infoxchange has reason to believe that disclosing the personal information and/or sensitive information is necessary to identify, contact or bring legal action against someone who may be causing injury to or interference with (either intentionally or unintentionally) Infoxchange's rights or property.
Infoxchange may share aggregate information provided by you through using this Portal, which is not personally identifiable, with Other Service Providers. This information may include usage and demographic data but will not include any personal information or any sensitive information.
Privacy, Confidentiality and Duty of Care Policy
Information about the way we collect, use, and share your personal and sensitive information provided to us or Infoxchange in using this Portal is aligned with the position of WA public sector.

Complaints and Feedback
You can make a complaint to us about how your personal and sensitive information provided in using this Portal has been managed.
